Knox Mindfulness does not disclose your personal information to overseas recipients. Knox Mindfulness has a Privacy Policy that is below. This policy provides guidelines on the collection, use, disclosure and security of your information. The Privacy Policy contains information on how you may request access to, and correction of, your personal information and how you may complain about a breach of your privacy and how we will deal with such a complaint. To ensure the process of quality treatment provision, information about your assessment results and progress will need to be collected and may be given to relevant other service providers, who are involved in your management. These may include your doctor, teachers, specialists, insurers, solicitors or employers.
Knox Mindfulness is committed to complying with the Privacy Act 1988 and the Australian Privacy Principles 2014 and the privacy provisions of all applicable legislation. This privacy policy covers all personal information we hold, that is, information, or an opinion about an individual, whose identity is apparent, or can be reasonably ascertained, from that information or opinion. This includes information we have collected from people through our office, over the phone and over the internet.
When we collect personal information from an individual, we will ensure that we do so in a fair manner and that we let the individual know where and how to contact our organisation. We will only collect information that is necessary for one or more of our functions or activities. We will advise individuals of the purpose for which their personal information is collected. If we collect sensitive information (as defined under the Act), we will treat it with the utmost security and confidentiality. We will ensure that it is not collected for any purposes, other than those for which we have obtained the individual's consent, unless the law requires otherwise, or other exceptional circumstances prevail as described under the Act. As Knox Mindfulness is committed to protecting the privacy of individuals, we will view unauthorized disclosure of, or access to, personal information by our employees, contractors, trainers or supervisors, as a serious breach of this policy. Appropriate action (which may include disciplinary or legal action) will be taken in such cases.
During the first session with Knox Mindfulness, all participants are given an easy to read document; this document includes the outline of how feedback, complaints and disputes will be handled.
In the front office is magazines, flyers, and a complaints / feedback form. Along with being given a copy at the beginning of their time with the service, they are easily accessible in the front office.
Depending on the reason for attendance of Counselling, a client will undertake 'progress reviews', at the start of the session when the counsellor asks about the goals, thoughts on counselling, they will also be given the complaints and feedback form. It will be explained to all clients that they can submit it anonymously but if they wish to know the outcome and steps put in place to rectify any issues, their details are therefore important.
When a participant fills in a complaints and feedback form, Knox Mindfulness must acknowledge and thank the participant or representative for taking the time to complete the form, along with advising them that it will be investigated and a written response will be provided within 7 days.

Where appropriate the service may be provided by telephone or videoconferencing. You are responsible for the costs associated with setting up the technology needed so you can access telehealth services. Knox Mindfulness will be responsible for the cost of the call to you and the cost associated with the platform used to conduct telehealth services.
To access telehealth consultations, you will need access to a quiet, private space; and the appropriate device, i.e. smartphone, laptop, iPad, computer, with a camera, microphone and speakers; and a reliable broadband internet connection.
The privacy of any form of communication via the internet is potentially vulnerable and limited by the security of the technology used. To support the security of your personal information this practice uses Phone Consult and Power Diary or Zoom which is compliant with the Australian standards for online security and encryption as well as HIPPA (for those accessing our service from overseas).
A telehealth consultation may be subject to limitations such as an unstable network connection which may affect the quality of the psychology session. In addition, there may be some services for which telehealth is not appropriate or effective. We will consider and discuss with you the appropriateness of ongoing telehealth sessions.
We will always provide individuals with a nil-cost way of contacting us to register a request to “opt -out” from receiving any product offers. Access to personal information Individuals will be able to access their personal information upon request. However, Knox Mindfulness may occasionally need to deny access to information in accordance with the exemptions contained in the Act.
Our goal is to protect the personal information collected by Knox Mindfulness and its associations. Personal information will be managed confidentially and securely and destroyed appropriately when no longer required. We will monitor and implement appropriate technical advances or management processes, to safeguard personal information.
